What Is PACE?
PACE stands for Program of All-Inclusive Care for the Elderly. It is a Medicare and Medicaid program that provides medical, personal, and social support through one coordinated care model.
PACE is designed for eligible adults age 55+ who need more support to continue living safely in their home and community.
At Habitat Health, PACE brings together care and daily support in one program. Services may include medical care, physical therapy, in-home care, transportation, center activities, social services, meals, nutrition, and 24/7 access to the Habitat Health care team.

Who May Qualify for Habitat Health PACE?
A person may qualify for Habitat Health PACE if they:
- Are age 55 or older
- Live in the Habitat Health service area
- Need support to continue living at home
- Can live safely in the community with PACE support
Eligibility is confirmed through the enrollment process. Habitat Health’s team can explain the program, check basic eligibility, offer a center tour, and walk families through the next steps.
Is Habitat Health Only for Kaiser Permanente Members?
No. You do not have to be a Kaiser Permanente member to join Habitat Health.
Habitat Health works in collaboration with Kaiser Permanente, and other providers beyond Kaiser Permanente are available in-network.
