Limited daily structure can increase the risk of functional decline, poor nutrition, medication lapses, and social isolation for older adults living at home. Habitat Health’s PACE program addresses this gap by providing structured attendance several days a week in a medically supported setting. Participants receive supervised exercise, care-plan-aligned meals, cognitive and social activities, and ongoing health monitoring that helps identify changes early and reinforce treatment plans between medical visits.

Habitat Health, in collaboration with Kaiser Permanente, officially celebrated the opening of its South Los Angeles Program of All-Inclusive Care for the Elderly (PACE) Center today with a ribbon-cutting ceremony attended by community leaders and healthcare partners. The event marked a significant milestone in expanding access to coordinated, whole-person care for older adults age 55 and over in one of Los Angeles County’s most medically underserved regions.

January marked the one-year anniversary of the Habitat Health Sacramento PACE Center. Over the past twelve months, Habitat Health’s collaboration with Kaiser Permanente has connected older adults to coordinated, centralized support that extends beyond traditional appointment-based care. Approximately 75% of Habitat Health participants are former KP members, reflecting the strength of this collaboration. As we enter year two, we remain focused on enabling older adults to maintain stability, independence, and connection within their communities. We commemorated the anniversary with a video highlighting the participants who made this possible. Watch the video featuring musical participant performances here.

The new center strengthens the local care infrastructure, delivering integrated medical, social, and in-home care to help older adults age with dignity in their communities.
