Habitat Health Sacramento Celebrates 1 Year Providing Care to Older Adults in the Community

January marked the one-year anniversary of the Habitat Health Sacramento PACE Center.

Over the past twelve months, Habitat Health’s collaboration with Kaiser Permanente has connected older adults to coordinated, centralized support that extends beyond traditional appointment-based care. Approximately 80% of Habitat Health participants were referred by Kaiser Permanente providers, reflecting the strength of this collaboration.

As we enter year two, we remain focused on enabling older adults to maintain stability, independence, and connection within their communities.
