How to Prepare for Your PACE Home Visit with Habitat Health

Have a home visit coming up? We're here to make it as easy as possible.

Before your visit, try to:
- Choose a quiet place where you can talk
- Have your medication bottles nearby
- Think about what has gotten harder day to day
- Invite a family member, if you’d like
- Don’t worry about cleaning or fixing anything
If your in-home visit is coming up, you don’t need to do much to get ready.
But these few simple things can make it easier.
Pick a good time and space
Try to choose a time when you’re not rushed. When your nurse arrives, it helps to sit somewhere quiet where you can talk comfortably.
There’s no need to set anything up. Just pick a spot where you feel at ease.
Have your medications nearby
If you can, gather your medication bottles before the visit. This helps make sure everything is accurate and nothing gets missed.
You don’t need to organize them—just having them close by is enough.
Think about your day-to-day
You don’t need to prepare answers, but it can help to think about your routine. What feels easy right now? What has gotten harder?
This is your chance to share what your days are really like so we can advocate for the support you deserve.
It’s okay if your home isn’t perfect
You don’t need to clean or change anything before the visit. Your nurse is there to understand your space, not judge it. Seeing things as they are helps us know how to support you.
Have someone with you (if you’d like)
You’re welcome to have a family member or caregiver there. Some people find it helpful to have someone beside them who can listen, take notes, or add details.
Be honest about what you need
This is the most important part. It’s okay to talk about what’s hard and how things have changed.
The more we understand, the better we can support you.

You don’t need to prepare perfectly. Just be ready to share your day, your routine, and what matters to you.
